Abstract
Healthcare institutions are urged to implement evidence-based procedures and clinical standards to guarantee that patients are treated effectively and safely. Since it directly affects patient outcomes, medication management is a crucial part of these strategies. This study evaluates the effectiveness of the medication management system at the casualty department of Katutura Intermediate Hospital in Windhoek, Namibia. The primary objectives were to assess the system’s effectiveness, identify common medication errors and their contributing factors, and explore the current medication management practices. A qualitative exploratory research design was employed, utilizing a semi-structured interview guide to collect data from a purposive sample of 30 healthcare professionals, including nurses and pharmacists. Data analysis was conducted using thematic analysis with NVivo software to identify recurring themes and patterns.
The findings indicate that the hospital's medication management system is well-structured and contributes to patient safety through effective storage, dispensing, and monitoring practices. However, several challenges were identified, including human errors, inadequate documentation, inconsistent adherence to verification procedures, and gaps in the medication error reporting process. Additionally, limited training opportunities and resource constraints were found to hinder the efficiency of the system. Despite these challenges, healthcare providers implement preventive measures, such as double-checking prescriptions and educating patients on medication usage and potential side effects. The study recommends the establishment of standardised protocols for medication verification and error reporting, continuous professional training on medication safety, and the adoption of technology-driven solutions to minimize errors and enhance efficiency. Strengthening interprofessional collaboration and optimising resource allocation were also identified as critical areas for improvement. The study concludes that with targeted interventions, the hospital can enhance its medication management practices, leading to improved patient safety, better resource utilisation, and overall healthcare efficiency.
